This assessment tool is for Performance Diagnosis. Most people work hard. Very few people work clearly. Before anything can change, we must establish where performance is currently breaking down at the level of clarity, ownership, and execution.

This diagnosis provides the evidence that will inform every decision you make this year.

HOW TO APPROACH THIS ASSESSMENT
As you complete the UNLEASH Performance Index™, answer every question through the lens of your current role at work and not how capable you believe you are or how much effort you invest or how you behave on your best days.

But how you operate consistently, what your manager and stakeholders actually experience and what results and patterns would support your answer.

If this assessment were reviewed by your manager, your team, or your board, would the score be defensible of you will become defensive?

EVIDENCE OVER INTENTION
This assessment is not scored on intention. It is scored on evidence. Therefore, ehen choosing your response, ask yourself:
• What happened in the last 90 days?
• What outcomes were delivered?
• What decisions were made or delayed?
• What behaviours were repeated?

If you cannot point to evidence, score lower. Honest scoring creates clarity. Inflated scoring creates false confidence.

For each statement, select the response that best reflects your actual behaviour at work over the last 90 days.

1 — Never
This does not happen in my work.

2 — Rarely
This happens occasionally, but it is not typical.

3 — Sometimes
This happens inconsistently or under limited conditions.

4 — Often
This happens regularly, with clear examples.

5 — Always
This happens consistently and reliably, with strong evidence.
